Switch Tables offers a guide for physicians and pharmacists in switching antidepressants and antipsychotics and in combining mood stabilizers and the conversion of benzodiazepines. It is a helpful tool to calculate the appropriate dosage when switching and/or combining psychopharmaceuticals and to learn the possible consequences of the switch and/or conversion.
Switch Tables aims to improve psychopharmacotherapy and medication safety for all patients. Switch Tables will be
updated on a regular basis.
